Case Manager - benefits navigation (remote)

Northwind Cross Health Group provides primary and specialty care in Omaha. We are hiring a Case Manager to take on benefits navigation. You would join a small organisation of about 176 people, working mostly from home. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for patient education. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with patient education. Working arrangement • Part-time, about 28 hours a week, afternoons. • Fully remote, with quarterly visits in Omaha. • We will tell you what to expect at each step of the hiring process.
