Case Manager - benefits navigation (remote)

Lumen Birch Care Services Partners supports families and individuals in Birmingham with personal care and coordination, and we need a Case Manager to lead our benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for client records. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with client records. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, spread across three days. • Fully remote, with a monthly team day in Birmingham. • We will tell you what to expect at each step of the hiring process.
