Case Manager - benefits navigation (remote)

We are growing our team at Bluepeak Sparrow Care Services Group and adding a Case Manager whose main responsibility will be benefits navigation. You would join a small organisation of about 46 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for visit scheduling. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, with flexible days. • Remote-friendly, with an occasional day on site in Buffalo. • Applications are reviewed on a rolling basis.
