Case Manager - benefits navigation (remote)

Harborlight Glen Health Group is a healthcare provider in Milwaukee with clinics and support teams. We are hiring a Case Manager to take on benefits navigation. Continuing education is supported. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for electronic records. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with electronic records. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • Fully remote, with a monthly team day in Milwaukee. • We will tell you what to expect at each step of the hiring process.
