Case Manager - benefits navigation (remote)

We are growing our team at Lumen Forge Care Services and adding a Case Manager whose main responsibility will be benefits navigation. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Help with applications and appointments for caregiver training. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • Remote-friendly, with a monthly team day in Pittsburgh. • We will tell you what to expect at each step of the hiring process.
