Case Manager - benefits navigation (remote)

We are growing our team at Clearpath Health Group and adding a Case Manager whose main responsibility will be benefits navigation. Continuing education is supported. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for electronic records. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Full-time, Monday to Friday. • Remote-friendly, with a monthly team day in New Orleans. • We will tell you what to expect at each step of the hiring process.
