Case Manager - benefits navigation (remote)

Bright West Health runs clinics and patient services around El Paso, and we need a Case Manager to lead our benefits navigation. Continuing education is supported. What you will do • Help with applications and appointments for patient scheduling. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with patient scheduling. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with clear deliverables from the first week. • Remote-friendly, with a monthly team day in El Paso. • Applications are reviewed on a rolling basis.
