Case Manager - benefits navigation (remote)

Northwind Stone Health Group is looking for a Case Manager. The job centres on benefits navigation for referrals, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for referrals. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with referrals.
