Case Manager - benefits navigation (remote)

Greenridge Rowan Health is looking for a Case Manager. The job centres on benefits navigation for referrals, and it would suit someone who likes steady, practical work. You would join a small organisation of about 32 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for referrals. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with referrals. Working arrangement • Part-time, about 16 hours a week, with flexible days. • Fully remote, with quarterly visits in San Jose. • We reply to every applicant.
