Case Manager - benefits navigation (remote)

Ironwood Keystone Health is a healthcare provider in Cincinnati with clinics and support teams. We are hiring a Case Manager to take on benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for electronic records. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with electronic records. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with clear deliverables from the first week. • Fully remote, with a monthly team day in Cincinnati. • Applications are reviewed on a rolling basis.
