Case Manager - benefits navigation (remote)

Stonegate Oak Care Services is looking for a Case Manager. The job centres on benefits navigation for caregiver training, and it would suit someone who likes steady, practical work. The work is personal and so is the culture. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for caregiver training. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with caregiver training. Working arrangement • Contract for four months, with the possibility of becoming permanent. • Fully remote, with quarterly visits in Minneapolis. • We reply to every applicant.
