Case Manager - benefits navigation (remote)

Bluepeak Quarry Care Services is a social-care provider serving older adults and people with disabilities around Charleston, and we need a Case Manager to lead our benefits navigation. You would join a small organisation of about 63 people, working mostly from home. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for caregiver training. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Contract for six months, with clear deliverables from the first week. • Remote-friendly, with an occasional day on site in Charleston. • Applications are reviewed on a rolling basis.
