Case Manager - benefits navigation (remote)

We are growing our team at Cedar Orchard Care Services and adding a Case Manager whose main responsibility will be benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for care plans. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Part-time, about 28 hours a week, afternoons. • Fully remote, with quarterly visits in Buffalo. • We will tell you what to expect at each step of the hiring process.
