Case Manager - benefits navigation (remote)

Bluepeak Basin Health Group is looking for a Case Manager. The job centres on benefits navigation for patient education, and it would suit someone who likes steady, practical work. You would join a small organisation of about 30 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for patient education. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with patient education. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • Fully remote, with an occasional day on site in Charleston. • We will tell you what to expect at each step of the hiring process.
