Case Manager - benefits navigation (remote)

Ironwood Birch Care Services is a social-care provider serving older adults and people with disabilities around Wichita. We are hiring a Case Manager to take on benefits navigation. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for care plans. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Contract for eight months, with clear deliverables from the first week. • Remote-friendly, with an occasional day on site in Wichita. • We will tell you what to expect at each step of the hiring process.
