Case Manager - benefits navigation (remote)

Harborlight Mill Care Services is looking for a Case Manager. The job centres on benefits navigation for care plans, and it would suit someone who likes steady, practical work. You would join a small organisation of about 40 people, working mostly from home. What you will do • Help with applications and appointments for care plans. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, afternoons. • Remote-friendly, with an occasional day on site in Virginia Beach. • We will tell you what to expect at each step of the hiring process.
