Case Manager - benefits navigation (remote)

Bright Clear Care Services provides in-home and community support in Jacksonville, and we need a Case Manager to lead our benefits navigation. You would join a small organisation of about 46 people, working mostly from home. What you will do • Help with applications and appointments for visit scheduling. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, mornings. • Fully remote, with a monthly team day in Jacksonville. • We reply to every applicant.
