Case Manager - benefits navigation (remote)

Stonegate Lantern Property Management Partners looks after rental homes across the Hartford area. We are hiring a Case Manager to take on benefits navigation. We are flexible about schedules. What you will do • Help with applications and appointments for resident events. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with resident events. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • Fully remote, with an occasional day on site in Hartford. • We will tell you what to expect at each step of the hiring process.
