Case Manager - benefits navigation (remote)

Northwind Bridge Care Services is a social-care provider serving older adults and people with disabilities around St. Louis, and we need a Case Manager to lead our benefits navigation. The work is personal and so is the culture. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for care plans. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, mornings. • Fully remote, with quarterly visits in St. Louis. • We reply to every applicant.
