Case Manager - benefits navigation (remote)

Nova Hollow Care Services is a social-care provider serving older adults and people with disabilities around Phoenix. We are hiring a Case Manager to take on benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for visit scheduling. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling. Working arrangement • Part-time, about 28 hours a week, afternoons. • Remote-friendly, with an occasional day on site in Phoenix. • We will tell you what to expect at each step of the hiring process.
