Case Manager - benefits navigation (remote)

Highbridge Health Group runs clinics and patient services around San Antonio. We are hiring a Case Manager to take on benefits navigation. Clinicians and administrators work side by side. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for patient education. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with patient education. • Bilingual English and Spanish. Working arrangement • Contract for four months, with clear deliverables from the first week. • Fully remote, with a monthly team day in San Antonio. • We reply to every applicant.
