Case Manager - benefits navigation (remote)

Silverline Cross Care Services supports families and individuals in Sacramento with personal care and coordination, and we need a Case Manager to lead our benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for family communication. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, spread across three days. • Remote-friendly, with a monthly team day in Sacramento. • We will tell you what to expect at each step of the hiring process.
