Case Manager - benefits navigation (remote)

Highbridge Stone Care Services provides in-home and community support in Raleigh. We are hiring a Case Manager to take on benefits navigation. You would join a small organisation of about 76 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for visit scheduling. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • Remote-friendly, with a monthly team day in Raleigh. • Applications are reviewed on a rolling basis.
