Case Manager - benefits navigation (remote)

Westbrook Glen Health is a healthcare provider in Oakland with clinics and support teams. We are hiring a Case Manager to take on benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for insurance authorisations. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with insurance authorisations. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • Remote-friendly, with a monthly team day in Oakland. • We will tell you what to expect at each step of the hiring process.
