Case Manager - benefits navigation (remote)

Harborlight Fern Care Services is looking for a Case Manager. The job centres on benefits navigation for client records, and it would suit someone who likes steady, practical work. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for client records. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with client records. Working arrangement • Part-time, about 20 hours a week, mornings. • Remote-friendly, with an occasional day on site in Charleston. • Applications are reviewed on a rolling basis.
