Case Manager - benefits navigation (remote)

Silverline Juniper Care Services provides in-home and community support in Boston, and we need a Case Manager to lead our benefits navigation. You would join a small organisation of about 13 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for care plans. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, mornings. • Fully remote, with a monthly team day in Boston. • Applications are reviewed on a rolling basis.
