Case Manager - benefits navigation (remote)

We are growing our team at Cedar Meridian Health and adding a Case Manager whose main responsibility will be benefits navigation. We are busy and kind, and we look after each other. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for patient education. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with patient education. • Bilingual English and Spanish. Working arrangement • Contract for twelve months, with a review at the end and a chance of extension. • Remote-friendly, with a monthly team day in San Antonio. • We reply to every applicant.
