Case Manager - benefits navigation (remote)

We are growing our team at Northwind Union Health and adding a Case Manager whose main responsibility will be benefits navigation. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for electronic records. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Full-time, with a steady weekday schedule. • Remote-friendly, with quarterly visits in San Antonio. • We will tell you what to expect at each step of the hiring process.
