Case Manager - benefits navigation (remote)

Northwind Lakeside Care Services provides in-home and community support in Cincinnati, and we need a Case Manager to lead our benefits navigation. You would join a small organisation of about 43 people, working mostly from home. What you will do • Help with applications and appointments for client records. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with client records. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • Remote-friendly, with a monthly team day in Cincinnati. • Applications are reviewed on a rolling basis.
