Case Manager - benefits navigation (remote)

Rivermark Ashton Care Services supports families and individuals in Atlanta with personal care and coordination. We are hiring a Case Manager to take on benefits navigation. The work is personal and so is the culture. What you will do • Help with applications and appointments for caregiver training. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Contract for twelve months, with clear deliverables from the first week. • Fully remote, with a monthly team day in Atlanta. • We reply to every applicant.
