Case Manager - benefits navigation (remote)

Ironwood Birch Care Services Group provides in-home and community support in Virginia Beach, and we need a Case Manager to lead our benefits navigation. You would join a small organisation of about 29 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for caregiver training. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • Fully remote, with quarterly visits in Virginia Beach. • We reply to every applicant.
