Case Manager - benefits navigation (remote)

We are growing our team at Amberfield Birch Care Services and adding a Case Manager whose main responsibility will be benefits navigation. We train and promote from within. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for family communication. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Contract for six months, with the possibility of becoming permanent. • Remote-friendly, with an occasional day on site in Oakland. • We reply to every applicant.
