Case Manager - benefits navigation (remote)

Stonegate Mill Care Services Partners supports families and individuals in Baltimore with personal care and coordination, and we need a Case Manager to lead our benefits navigation. Teams support each other, especially on hard days. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for client records. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with client records. Working arrangement • Part-time, about 20 hours a week, mornings. • Fully remote, with quarterly visits in Baltimore. • We will tell you what to expect at each step of the hiring process.
