Case Manager - benefits navigation (remote)

We are growing our team at Amberfield Bridge Health and adding a Case Manager whose main responsibility will be benefits navigation. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for clinic supplies. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish. Working arrangement • Contract for nine months, with the possibility of becoming permanent. • Remote-friendly, with a monthly team day in Raleigh. • Applications are reviewed on a rolling basis.
