Case Manager - benefits navigation (remote)

Lumen Fern Care Services Partners is a social-care provider serving older adults and people with disabilities around Wichita, and we need a Case Manager to lead our benefits navigation. You would join a small organisation of about 24 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for licensing. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with licensing. Working arrangement • Part-time, about 24 hours a week, with flexible days. • Fully remote, with an occasional day on site in Wichita. • We will tell you what to expect at each step of the hiring process.
