Case Manager - benefits navigation (remote)

Bright Umber Health runs clinics and patient services around Virginia Beach. We are hiring a Case Manager to take on benefits navigation. You would join a small organisation of about 17 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for referrals. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with referrals. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • Fully remote, with quarterly visits in Virginia Beach. • Applications are reviewed on a rolling basis.
