Case Manager - benefits navigation (remote)

We are growing our team at Lumen Fern Care Services and adding a Case Manager whose main responsibility will be benefits navigation. We train and promote from within. What you will do • Help with applications and appointments for caregiver training. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with caregiver training. Working arrangement • Part-time, about 28 hours a week, mornings. • Fully remote, with a monthly team day in Fresno. • We reply to every applicant.
