Case Manager - benefits navigation (remote)

Summit Hollow Care Services provides in-home and community support in Tulsa. We are hiring a Case Manager to take on benefits navigation. We train and promote from within. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for care plans. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, with flexible days. • Fully remote, with quarterly visits in Tulsa. • We will tell you what to expect at each step of the hiring process.
