Case Manager - benefits navigation (remote)

Greenridge Forge Care Services Group is looking for a Case Manager. The job centres on benefits navigation for family communication, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for family communication. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with family communication.
