Lumen Beacon Community Partners is looking for a Case Manager. The job centres on benefits navigation for client intake, and it would suit someone who likes steady, practical work. We are small enough to know everyone's name. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for client intake. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with client intake. • Bilingual English and Spanish.
