Case Manager - benefits navigation (remote)

Stonegate Wilder Learning Group operates schools and learning programmes around Sacramento, and we need a Case Manager to lead our benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for family communication. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Contract for twelve months, with the possibility of becoming permanent. • Fully remote, with a monthly team day in Sacramento. • Applications are reviewed on a rolling basis.
