Case Manager - benefits navigation (remote)

Highbridge Birch Care Services supports families and individuals in Portland with personal care and coordination, and we need a Case Manager to lead our benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for licensing. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with licensing. Working arrangement • Part-time, about 20 hours a week, afternoons. • Remote-friendly, with an occasional day on site in Portland. • We will tell you what to expect at each step of the hiring process.
