Case Manager - benefits navigation (Raleigh, NC)

Greenridge Linden Health Group runs clinics and patient services around Raleigh. We are hiring a Case Manager to take on benefits navigation. We are busy and kind, and we look after each other. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for electronic records. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with electronic records. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Raleigh. • We will tell you what to expect at each step of the hiring process.
