Case Manager - benefits navigation (Raleigh, NC)

Bluepeak Hollow Health Group provides primary and specialty care in Raleigh, and we need a Case Manager to lead our benefits navigation. You would join a small organisation of about 58 people based in Raleigh. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for electronic records. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in Raleigh. • We will tell you what to expect at each step of the hiring process.
