Case Manager - benefits navigation (Raleigh, NC)

Bluepeak Sparrow Care Services Group provides in-home and community support in Raleigh, and we need a Case Manager to lead our benefits navigation. You would join a small organisation of about 26 people based in Raleigh. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for visit scheduling. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Contract for four months, with the possibility of becoming permanent. • On site in Raleigh. • Applications are reviewed on a rolling basis.
