Case Manager - benefits navigation (Pittsburgh, PA)

Westbrook Drift Care Services is a social-care provider serving older adults and people with disabilities around Pittsburgh. We are hiring a Case Manager to take on benefits navigation. You would join a large organisation of more than 1,000 people based in Pittsburgh. What you will do • Help with applications and appointments for client records. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with client records. • Bilingual English and Spanish. Working arrangement • Contract for nine months, with the possibility of becoming permanent. • On site in Pittsburgh. • Applications are reviewed on a rolling basis.
