Case Manager - benefits navigation (Pittsburgh, PA)

Cedar Bridge Care Services Group supports families and individuals in Pittsburgh with personal care and coordination, and we need a Case Manager to lead our benefits navigation. You would join a small organisation of about 44 people based in Pittsburgh. What you will do • Help with applications and appointments for licensing. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with licensing. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in Pittsburgh. • Applications are reviewed on a rolling basis.
