Case Manager - benefits navigation (Phoenix, AZ)

Lumen Cedar Health Group is a healthcare provider in Phoenix with clinics and support teams. We are hiring a Case Manager to take on benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for referrals. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with referrals. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Phoenix. • Applications are reviewed on a rolling basis.
