Clearpath Drift Health is looking for a Case Manager. The job centres on benefits navigation for patient scheduling, and it would suit someone who likes steady, practical work. You would join a small organisation of about 59 people based in Phoenix. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for patient scheduling. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with patient scheduling. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • On site in Phoenix, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
