Case Manager - benefits navigation (Philadelphia, PA)

We are growing our team at Ironwood Rowan Health Group and adding a Case Manager whose main responsibility will be benefits navigation. You would join a small organisation of about 91 people based in Philadelphia. What you will do • Help with applications and appointments for insurance authorisations. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations. Working arrangement • Part-time, about 24 hours a week, afternoons. • On site in Philadelphia. • We reply to every applicant.
